-Discharge planning means a smoother transition resulting in lower rehospitalization rates.

-Lab work, medical testing, church, religious services, shopping/picking up medication, and family events (weddings, graduations, etc.)
- For improved communication Collective can participate in appointments with your loved ones to relay information to and from medical providers.
- Alzheimers/ Demential Program
